Local Health Literacy/Patient Information Primary Health Care

Review of HealthPathways – Patient Information

This report reviews the patient information provided in the clinical pathways on HealthPathways, an online tool for primary health teams. The Health Care Consumer’s Association was asked to review this information so it meets an acceptable level of coverage and completeness for consumers and carers, particularly regarding condition-specific information, fact sheets and support services available in the ACT and south-eastern NSW. The project aimed to:

  • assess resources and links in the ‘Patient Information’ sections for information
    gaps across all localised clinical and resource pathways
  • provide missing information resources, links and contact information for all live
    pathways (as at 2 June 2017).
