In May 2023 HCCA created the Chronic Conditions Network in response to a clearly identified need for a new network of community organisations that support people living with chronic conditions.
The primary goal of the Chronic Conditions Network is to “identify key issues affecting consumers with chronic conditions and strengthen collaboration on systemic advocacy in relation to these issues”.
The processes employed to achieve this primary goal at monthly networking meetings include:
- Awareness – Create awareness of chronic condition community organisations operating in the ACT and surrounding region, and what services they offer.
- Connection – Allow organisations to connect with one another to form relationships, share information and training, and to reduce duplication of effort.
- Collaboration – Provide opportunities for organisations to collaborate with each other to share experience and work together identifying issues affecting the sector.
- Coordination – Support coordinated advocacy and engagement which delivers impactful messaging to health services on identified issues of importance.
- Partnership – Encourage organisations to partner with each other on policy submissions, joint statements, funding applications and other initiatives.
